About the service
The Practice Nurse runs diabetes managements clinics. You will be notified when your annual or interim review is due. Please arrange your blood tests so the results are available for your consultation with the Practice Nurse.
The reviews are important to ensure your diabetes is managed effectively, reviewing your overall health, blood tests, lifestyle and offer the support as needed.
